Memorial Hospital resuming Medicare annual wellness visits

Memorial Hospital and Health Care Center providers are now resuming annual wellness visits for Medicare patients. All Memorial Hospital clinics have implemented additional safety measures recommended by the Centers for Disease Control (CDC) and the Indiana State Department of Health to ensure the safety of patients.

To schedule an appointment for an annual wellness visit, please call the office of your primary care provider. If you do not have a provider, call Memorial Hospital’s physician referral line at 812-996-CARE (2273).
